#1. FILE AN EMPLOYER INCIDENT REPORT IMMEDIATELY

Complete an incident report (no matter how minor the incident may be). They can be found on -line if there are none pre-printed in your area. Be sure to include: all parts of body involved, where and when it happened and what caused the injury. BE SPECIFIC and provide witnesses full names.

AN EMPLOYER INCIDENT REPORT DOES NOT START A WSIB CLAIM!!!

#2. BE CONSISTENT 

Be consistent in reporting, as you may have to report your incident/injury many different times, i.e. incident report, form 6, WSIB case manager, etc.

 

#3. SEE YOUR DOCTOR

Visit your health care provider such as a physician, nurse practitioner, physiotherapist, chiropractor, local emergency room, etc. Make sure they are aware that this is a work-related injury.
